    What it measures

    The ADHD-RS-5 is an 18-item scale that mirrors the DSM-5 criteria for ADHD, covering nine inattention and nine hyperactivity-impulsivity symptoms, completed by a parent or teacher for children and adolescents.

    How scoring works

    Each item is rated 0 (never or rarely) to 3 (very often). Inattention, hyperactivity-impulsivity and total raw scores are compared with age- and gender-based percentile norms.

    The ADHD-RS-5 is copyright Guilford Press. The purchase licence only permits an individual clinician to photocopy forms for their own patients; it does not cover resale, redistribution or embedding in a commercial product, which needs a separate commercial digital licence from Guilford.
